Palliative Care:
Facing a life-limiting illness is difficult for participants and their families alike. Our palliative care service is built around comfort, not just clinical need, helping clients live as fully and peacefully as possible in familiar surroundings, close to the people who matter most.
Pain and symptom management sits at the heart of what we do. Our nurses closely monitor and manage pain, breathlessness, nausea and other symptoms, working to keep clients as comfortable as possible at every stage.
Emotional support for participants and families is just as important as clinical care. Facing end-of-life is rarely a purely physical experience, and our team takes time to listen, reassure and support both clients and the people caring for them, through difficult conversations and difficult days.
Comfort-focused, dignity-centred care guides every visit. We support clients to remain in control of their own care wherever possible, respecting their wishes, routines and personal choices, and treating every individual with the respect they deserve.
Collaboration with palliative care teams ensures continuity and consistency. We work closely with treating specialists, GPs and specialist palliative care services to make sure care at home reflects the broader care plan, so families aren’t left coordinating between services on their own.
Post-Surgical Care:
Recovery after surgery often continues well beyond the hospital doors. Our post-surgical nursing service supports clients through this critical period with wound care and dressing changes, medication management, monitoring for signs of infection or complications, and mobility support as clients regain strength and independence.
By catching concerns early and keeping treating doctors informed, our nurses help reduce the risk of complications and unplanned hospital readmissions, allowing clients to heal safely and comfortably at home.
